What To Expect
New patients will be required to complete the intake forms and psychological screening questionnaires prior to scheduling their appointment. During your initial appointment, you will be asked detailed questions about your symptoms and medical history. This will help rule out other possible conditions, accurately form a diagnosis using the DSM-5, and create a holistic treatment plan for you. May order labs or other tests, if determined necessary, or if needed prior to starting medication. You will have the opportunity to ask questions and express your concerns and discuss your treatment plan.
Confidentiality
Your records are confidential and will not be released without your written consent, with some exceptions; exceptions include if you are a threat to yourself or others or through a subpoena. As a mandated reporter, I have a legal duty to report any suspicions of child, elder, or disabled person abuse.
Emergencies and Crisis
We are an outpatient clinic, not an emergency clinic. If you are having an emergency or in crisis, please call 911 or contact one of the emergency/crisis resources provided. This office does not have after-hours, emergency, or Urgent Care hours.
Court
We are not involved or contracted by any court system. We do not provide court-ordered services related to child custody, family court, or other matters.
